Safeguarding Concern Form Generator

Create a clear, printable record of a safeguarding concern to pass securely to the person responsible for safeguarding in your organisation.

A recording tool, not an assessmentHow to use this form

This tool records information. It does not assess a concern, decide whether abuse occurred or tell you what action to take. Follow your organisation's safeguarding policy, safeguarding lead process and local safeguarding procedures. This is not an official government form or legal advice.

Your organisation may call its safeguarding lead the Designated Safeguarding Lead (DSL). The form uses “safeguarding lead / person responsible for safeguarding”.

Safeguarding concern information

1. About this record

Use not known if appropriate. Do not guess missing information.

2. Child / young person

Include only what is useful to identify this record. Leave information blank if not known.

3. Concern

Distinguish what you personally saw or heard, what another person reported, and any opinion. Record facts and their source; do not diagnose or investigate.

Record their words as accurately as possible. Do not ask additional questions simply to complete this form. Do not promise secrecy; follow your safeguarding procedure.

Include only information relevant to this concern. Do not add medical or additional-needs information merely as general identification.

4. Other people relevant to the concern

Name, role or relationship only where relevant. Distinguish who observed something from who reported it.

Record relevant name, role or relationship neutrally. This is not a finding about their behaviour.

5. Immediate actions

Record only actions already taken. Follow-up is for your safeguarding lead to decide under your procedure.

Actions already taken (select any that apply)

For example, who was contacted, when and what happened. This form does not decide what should happen next.

Communication depends on the circumstances and your safeguarding procedure. If informing a parent could increase risk or interfere with safeguarding action, follow that procedure and seek advice from your safeguarding lead. Do not rely on this tool to decide.

6. Generate your PDF

Check your entries before generating the record. Only the concern account is required; leave unknown optional information blank.

Clears entries, the preview and generated PDF from this page. Downloaded and printed copies remain on your device or printer.
