Risk Assessment Builder

Identify hazards, record the controls you have in place and create a printable risk assessment for your activity.

Risk assessment details and hazards

1. About the assessment

2. Identify hazards

Every activity and venue is different. These prompts are not exhaustive. Add any other hazards relevant to your session.

Common things to consider
Activity-specific things to consider

Football / field sports

Swimming

Gymnastics

Dance

Martial arts

Outdoor activities

Adding a prompt fills only the hazard description. It does not add controls or confirm that anything is in place.

No hazards added yet.

3. Review actions

For each hazard, record what you actually do now and any further action needed. You can assign a person or role and a target date in each hazard card. Follow through on the actions; recording them does not implement them.

Risk scoring is optional. Some organisations use likelihood × severity to help prioritise identified risks. Your organisation may use a different method. This tool’s 1–5 scales are not an official HSE matrix and scores do not decide whether an activity is safe.

4. Create printable assessment

Review your details, hazards, controls and actions before printing. Unknown optional details can stay blank. If there are no existing controls, record that explicitly rather than describing planned controls as already in place.

Clears entries and the review from this page. Nothing is saved for later.
